Practice Nurse

 We currently have a vacancy for an experienced and enthusiastic Practice Nurse to join our Nursing Team.  You should be competent in core duties such as general treatment room skills, including immunisations and cervical screening and ideally have experience of chronic disease management (COPD/asthma/diabetes) although the opportunity for further training could be arranged for the successful candidate. We are also looking for a part-time receptionist to join our busy team. The hours will be Monday 8am to 2pm, Thursday 8am to 2pm and Friday 8am to 2pm with additional sickness and holiday cover when required. The ideal candidate will have excellent communication skills, be hardworking, reliable and able to work on their own initiative, as well as a team player. The reception staff will explain how to register as a patient. We accept new patient registrations from 11am onwards each day.

If you have any complex medical history or long term conditions such as Diabetes, Heart Failure or COPD please come in for a new patient check-up within six weeks of joining the practice. This is a double appointment with one of the doctors and allows us to get an understanding of your medical history prior to receiving your full medical record from your previous practice.



Registering with the practice 

register_nowIf you wish to speed up your registration with the practice, you may like to print off and complete the GMS1 form below. Please complete the form, along with the New Patient questionnaire, listed below, and bring them both along to the practice when you wish to register. We will also need to see photographic proof of your identity, either  a photo driving licence or your passport, along with proof of your address. This may be a utility bill, mortgage/bank statement or property rental agreement.

Since mornings are our busiest time, we ask that new patients come in to register after 11am each day. This not only helps us but will mean that you shouldn't have to wait for too long to be seen by one of the reception staff.

When you register with us, you will be allocated a named, accountable GP. Although this doctor will have overall responsibility for the care and support that we provide to you, this does not prevent you from seeing any other doctor in the practice of your choice.

Similarly since none of the GPs here work five days a week, if your named doctor isn’t working on the day that you want to see or speak to someone, you can speak to any other doctor here as they all have full access to your medical records. If you want to know who your named GP is, please just ask a member of staff on reception.

Please note that all registration requests will be subject to acceptance by the doctors. The practice has a dedicated catchment area and we may be unable to accept patients from outside this area except under special circumstances and upon agreement with the doctor.

When you register you will also be asked to fill out a New Patient questionnaire. This is because it can take a considerable time for us to receive your medical records. In order to speed up your registration, there is an online version of this form too, which you can fill out in advance and bring along with you.
